Simple Ordering Details: For
How many rooms or televisions do you want to connect to your satellite system? CChoose the system for which you are interested. When you speak with an installation professional or service representative, they will learn your viewing and equipment preferences then guide you in choosing the correct equipment. The receiver choices you make on this order form do not need to be your final choices. Note: Offer includes a maximum of 4 receivers (TV's) and that there is a limit of 2 HD receivers and 1 HD/DVR. In addition to a $5.00 per month equipment rental fee per receiver (above the
first), a $4.99 per month additional outlet programming access fee will be
charged for each dual-tuner (DISH 322, DISH Player-DVR 625, or DISH Player-DVR
942) receiver. However, the monthly $4.99 programming access fee will be waived
for each dual-tuner receiver that is continuously connected to the customer's
phone line. There is a $250 upgrade fee for the DISH
Player-DVR 942. This upgrade fee will be charged at the time of account
activation.

must subscribe to a qualifying programming
package; either DISH Latino, DISH Latino Dos,
DISH Latino Max, America's Top 60, America's

Package to purchase international programming. 1You may order DISH Network HD Pak by itself. However, VOOM Originals HD Pak is only available with a subscription to DISH Network HD Pak. Certain HD channels will require an additional dish antenna at a cost of $100 at the time of initial installatio 18 Months - Includes Home Protection Plan* $49.99 credit on the first bill requires an 18-month commitment to Digital Home Advantage. If qualifying service is terminated prior to end of 18 months, a cancellation fee equal to the lesser of $240 or $13.33 per month for each cancelled month of service will apply. With an 18 month commitment, the $49.99 activation fee will be credited on your first bill.
